Overview

Community Health Workers (CHW) are  frontline public health workers who are trusted members of the community.  CHWs have a close understanding of the needs of the community and are able to serve as a link between health and social services and the community to help facilitate greater access to those much needed services. Community Health Workers provide a wide range of services including, health education and outreach, care management, social support and advocacy.   

We educate community health workers to address the social determinants of health, and promote health equity and social justice within the community they serve. Our students undergo training in enhancing individual and community capacities, promoting health and preventing diseases, cultural mediation, and community based advocacy.

Our Community Health Worker certificate program includes classroom instruction and field experience that emphasizes the Community Health Worker core competencies in public health, communication skills, interpersonal and relationship building, service coordination and navigation skills, capacity building and advocacy, education and facilitation skills, assessment of the client and the community, outreach, professional skills, evaluation, and research. We offer continuous support and mentorship to students for up to six months after course completion to enhance professional and workforce capabilities.


Program Outcomes

Upon successful completion of this program, participants will be prepared to:

  1. Identify basic health needs and evaluate health and social resources within the local community

  2. Demonstrate professional skills and job readiness of a Community Health Worker as outlined by the Community Health Worker Core Consensus Project (CP3) and the needs of the local community.

  • Advocate/Aide/Ambassador, Care Coordinator, Case Worker/Manager, Counselor, Health/Patient Navigators, Health Educator/Specialist, Outreach/Support

  • Urgent Care/Clinics/Hospitals, Mental Health Facilities, Government Agencies, Community Health Centers, Community or Faith-based Organizations
